SAGE is built around a simple monthly membership that gives you more direct access to your provider, longer visits, and a more personal relationship with your care team.

What is Direct Primary Care?

Direct Primary Care, often called DPC, is a membership-based healthcare model focused on access, relationship, and simplicity. Instead of billing insurance for every routine primary care visit, members pay a predictable monthly fee for everyday care.

What is SAGE Primary Health?

SAGE Primary Health is a relationship-based primary care practice designed to make healthcare feel more personal, accessible, and transparent. Members receive direct provider access, longer visits, same or next-day availability when possible, and routine primary care without per-visit copays.

How is SAGE different from traditional primary care?

Traditional care can feel rushed, difficult to access, and confusing financially. SAGE limits the membership panel so providers have more time for each patient. That means better communication, more thoughtful visits, and care that is centered around your actual needs.

What does the membership include?

Your membership includes routine primary care visits, direct communication with your care team, preventive care, chronic condition support, follow-up care, and guidance for common health concerns. Certain labs, medications, procedures, imaging, and outside services may carry additional costs.

Do I need health insurance?

SAGE does not replace major medical insurance. Many members keep insurance for emergencies, hospital care, surgery, specialists, imaging, and catastrophic needs. SAGE is designed to improve access to everyday primary care and can work alongside your insurance plan.

Does SAGE accept health insurance?

SAGE operates outside the traditional insurance billing model for membership services. This keeps routine care simple and transparent. Instead of billing insurance for primary care visits, members pay a monthly fee directly to the practice.

Can I join if I already have insurance?

Yes. Many members use SAGE alongside their existing insurance. Insurance can remain in place for major medical needs, while SAGE becomes your first stop for everyday care, questions, follow-ups, and preventive health.

Can I join if I do not have insurance?

Yes. SAGE can be a valuable option for uninsured patients who want predictable access to primary care. We still recommend major medical coverage when possible for emergencies, hospitalization, and specialty care.

Will I benefit if I do not go to the doctor often?

Yes. Direct access to a provider is valuable even when you are healthy. SAGE helps you stay proactive with preventive care, annual wellness planning, early trend detection, and fast answers when something comes up.

Am I locked into a long-term contract?

No. SAGE is designed to be simple and flexible. Membership cancellation terms should be confirmed with the clinic, but the goal is to avoid complicated long-term commitments.

Can Medicare patients join?

Medicare patients may be able to join a Direct Primary Care membership, depending on the practice’s policies and required agreements. Medicare may still apply to covered outside services such as hospital care, imaging, labs, or specialist services when appropriate.

What if I need care while I am away from home?

Members can often contact the care team directly for guidance while traveling. When appropriate, your provider may help with questions, next steps, medication guidance, or whether you should seek urgent or emergency care locally.

Do appointments start on time?

SAGE is built around a smaller patient panel and a more intentional schedule. While healthcare needs can vary, the goal is to provide timely appointments without the rushed, overbooked feeling common in traditional care.

Do you treat children and families?

SAGE can be structured for individuals, families, and employer groups. Age ranges, family enrollment policies, and child membership details should be confirmed directly with the clinic before joining.

What services are not included?

Hospitalization, emergency room care, surgeries, specialist visits, advanced imaging, and certain outside labs or medications are generally not included in a Direct Primary Care membership. Those services are typically handled through insurance or separate cash-pay arrangements.
